Provide and facilitate the delivery of CAP case management to disabled adults and medically fragile children living in a private residence who are at risk of institutionalization and whose health, safety, and well-being can be maintained within budget restrictions set by the Division of Medical Assistance. The CAP Clinical Supervisor provides support to case managers, individuals being served, as well as the family and the community in which the beneficiary lives. A CAP Clinical Supervisor will coordinate with staff to assure quality case management, and ensure that methods and practices comply with current accepted approaches for case management. The CAP Clinical Supervisor will provide direct case management, consultation, training and development to staff, reviewing and approving case management interventions and serving as a liaison with state agencies.
The Community Alternative Program for Children is a Community Based Services Waiver designed to provide an alternative placement in a nursing facility for beneficiaries who prefer to remain in his or her primary private residences and would be at risk of institutionalization without these services. We work with individuals to establish the person-centered plan and to coordinate care and services including personal care, waiver supplies, home modifications and educational support.
Responsibilities:
- Processing wait list from initial intake (referral) to assessment assignment.
- Balance current case management caseloads to case manager ability/request.
- Assist with case load management during times of transition and vacations while waiting for case management assignment continuity of care.
- Reviews case management notes for accuracy, content, and approves the notes as needed.
- Provide clinical oversight and ongoing training for the Case Managers
- Develop, coordinate and provide regional and individual in-service trainings to improve quality services for the program.
- Train new hire staff to provide direct clinical support and to sign off/approve clinical documentation including but not limited to assessments, plans of care, revisions, eCAP notes, CIR, RAI until staff meet case management qualifications.
- Assist with the development and monitoring of internal Quality Improvement programs for case management review and evaluate programs, ensuring best practice and compliance with all federal and state laws, policies and regulations.
- Provide LAR (Internal Local Approval Review) for all Initials, CNR's, CIS/COS, POCR within timeline provided by DHB and ensure all required uploads are present.
- Arrange, plan agenda and implement unit meetings as requested by leadership team.
- Provides oversight and monitoring to ensure services are being delivered as approved by DHB and as stated on the service authorization.
- Present updates/problematic situations/reports to leadership team as requested
- Assures that required DHB reports regarding customer and staff outcomes are completed and submitted in a timely manner as required by RHA Health Services and participate on the RHA CAP Leadership Team. Participate in all DHB required trainings
- Goal is to ensure that all Case Managers are providing and facilitating all clients to the fullest capacity.
Requirements:
- Bachelor's degree in social work from an accredited school of social work, and five years of directly related experience of community experience (preferably CAP case management) in the health or medical field directly related to homecare, long-term care, or personal care;
- Registered nurse who holds a current North Carolina license, two-year or four-year degree, five years case management (preferably CAP case management) in homecare, long-term care, personal care or related work experience.
